Not all care is created equal. We help families distinguish between different levels of support to ensure the right fit for their loved one's physical condition.

Activities of Daily Living (ADLs) vs. IADLs

• Personal Care (ADLs): Hands-on physical assistance. This includes bathing, grooming, dressing, and transferring safely from a bed to a chair.
• Companion/Homemaking (IADLs): Support for independent living. This includes meal preparation, light housekeeping, laundry, and money management.

Specialized Health Monitoring

For those with complex medical needs, we offer Special Maintenance that often requires a physician’s order:

• Monitoring Vital Signs: Consistent tracking of health status to prevent hospital readmissions.
• Clinical Support: Specialized programs for Wound Care*, Range of Motion (ROM), and Bowel/Bladder Programs.

Understanding how to fund care is often the biggest hurdle for families. SuBon participates in major Virginia programs to make "aging in place" accessible.

The Medicaid Waiver Guide

• CCC Plus Waiver: Designed to help older adults and those with complex medical needs stay home longer instead of moving to a nursing home.
• DD Waiver: Focused on empowering individuals with developmental disabilities to access the community, work, and school.
• Managed Care (MCO): SuBon is a proud participating provider for Sentara and Humana.
• Fee-for-Service (FFS): We accept traditional Medicaid for those not yet enrolled in an MCO.
