Senior Care Resources

Senior Care Resources

1. Understanding the Spectrum of Care

Not all care is created equal. We help families distinguish between different levels of support to ensure the right fit for their loved one's physical condition.

Activities of Daily Living (ADLs) vs. IADLs

  • Personal Care (ADLs): Hands-on physical assistance. This includes bathing, grooming, dressing, and transferring safely from a bed to a chair.
  • Companion/Homemaking (IADLs): Support for independent living. This includes meal preparation, light housekeeping, laundry, and money management.

Specialized Health Monitoring

For those with complex medical needs, we offer Special Maintenance that often requires a physician’s order:

  • Monitoring Vital Signs: Consistent tracking of health status to prevent hospital readmissions.
  • Clinical Support: Specialized programs for Wound Care*, Range of Motion (ROM), and Bowel/Bladder Programs.

2. Navigating Medicaid Waivers & Insurance

Understanding how to fund care is often the biggest hurdle for families. SuBon participates in major Virginia programs to make "aging in place" accessible.

The Medicaid Waiver Guide

  • CCC Plus Waiver: Designed to help older adults and those with complex medical needs stay home longer instead of moving to a nursing home.
  • DD Waiver: Focused on empowering individuals with developmental disabilities to access the community, work, and school.
  • Managed Care (MCO): SuBon is a proud participating provider for Sentara and Humana.
  • Fee-for-Service (FFS): We accept traditional Medicaid for those not yet enrolled in an MCO.

3. Local Expertise: Our Service Areas

We believe the best care comes from neighbors. Click the links below to learn more about the specific neighborhoods we serve in your area:

  • Chantilly & South Riding: Serving families near StoneSprings Hospital and the Dulles corridor.
  • Centreville: Supporting residents in Little Rocky Run, Sully Station, and beyond.
  • Herndon & Reston: Expert care for Franklin Farm, Oak Hill, and Floris residents.
  • Ashburn & Sterling: Dedicated support for Broadlands, Brambleton, and Cascades.
  • Aldie: Serving the growing communities of Willowsford and Stone Ridge.

4. Professional & Family Toolkits

We provide the "blueprints" for successful care at home.

  • The SuBon Caregiver Standard: Learn about our rigorous vetting, background checks, and specialized training.
  • Respite Care for Families: A resource for primary caregivers to learn how to prevent burnout while we manage medication reminders and safety.
  • Referral Partner Portal: Information for discharge planners and social workers on how to send a secure referral to SuBon.

5. External Resources & Advocacy

For further education on aging and disability rights in Virginia, we recommend the following external resources:

  • Virginia Department of Medical Assistance Services (DMAS): The official source for Medicaid Waiver information.
  • Fairfax County Area Agency on Aging: Local resources for seniors in our service area.
  • AARP Family Caregiving Guide: A national resource for managing the emotional and financial aspects of care.
